Practitioner Services update the Community Health Index (CHI) based on data quality exercises undertaken about the patients of Scotland.

Data held on the Community Health Index (CHI) needs to be up-to-date and accurate.

Data quality exercises are carried out to keep patient demographic details, as well as registration status, current.

Patient are directly contacted for some exercises. Others require input from GP Practices. These may be for individual patients or a group of patients.

Up-to-date information is shared throughout the NHS to ensure patients can be positively identified and records kept accurate.
