How to handle a patient request to access their records

When a patient requests access to a medical record, it’s subject to a clinical review.

To ensure this happens the request should be made to the current or last GP practice where the patient was registered.

If a GP practice receives a request where they don’t currently hold the record, it can be requested from Practitioner Services who’ll retrieve and return it.

If in a rare case the practice is unable to provide access, for example, due to an ongoing legal case, the request may be forwarded to the NHS Board Medical Director for approval.
